Key Responsibilities

• Evaluate and treat patients with speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans based on patient needs and goals
• Provide services across multiple facilities within the same city
• Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to support optimal patient outcomes

Required Qualifications

• Active Speech Language Pathologist (SLP) license in the state of Illinois
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred or eligibility required
• Ability to work across multiple nearby facilities within the same city
• Strong clinical documentation and communication skills

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Experience managing a caseload across multiple sites
• Familiarity with dysphagia evaluation and treatment protocols
