RN Case Manager(part time-full time)

Rn Case Managers

Rn case managers are a critical part of the agency and must have experience working in the home health setting. We are looking for individuals that are versatile and dependable. Individuals must have the following:

• Must hold a current license as a registered nurse from the virginia state board of nursing.
• Must have experience working in a home care agency.

Job duties:

• Complete initial and ongoing comprehensive assessments for client admissions.
• Create patient plan of care and reevaluate as necessary.
• Identify any additional patient needs and facilitate appropriate referrals.
• Complete clinical nurse notes in their entirety.
• Facilitate monthly home visits for skilled care.
• Additional duties as assigned.

Job types: full-time, part-time, contract

Schedule:

Monday to Friday

On call

Weekends as needed

Work location: in person
