About the Role: As a RN at Vestal Park Rehabilitation and Nursing Center, you will be responsible for providing direct patient care, assessing patient needs, and developing individualized treatment plans. You will work closely with our interdisciplinary team to ensure that our residents receive the best possible care, supporting their physical, emotional, and psychosocial well-being. Responsibilities: Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions Assess patient needs and develop individualized treatment plans in collaboration with the interdisciplinary team Develop and implement patient care plans, including setting goals and objectives Communicate effectively with patients, families, and healthcare teams to ensure a cohesive and effective care approach Participate in quality improvement initiatives and clinical training programs to stay up-to-date with best practices and industry standards Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes Minimum Requirements: RN licensure in the State of New York Current CPR certification Ability to work effectively in a fast-paced, dynamic environment Excellent communication and interpersonal skills #J-18808-Ljbffr
