Your future role at a glance Location: Indianapolis, IN Facility: Ascension Medical Group Clinic Specialty: Cardiology Schedule: Monday - Friday 7am -5pm, partially remote Salary range: $40.41 - $57.05 How you'll make an impact in this role Implement and monitor patient care plans by expertly assessing and evaluating symptom trends over the phone, updating the healthcare team, and providing immediate clinical guidance. Execute physician and nursing orders with precision, acting as the vital link between the Cardiology team and the patient to ensure treatment plans are followed accurately at home. Coordinate seamless care transitions by managing discharge planning and providing specialized education on heart health, medication adherence, and post-procedural recovery. Lead clinical triage by utilizing advanced cardiac assessment skills to determine the safest and most effective level of care for patients experiencing cardiovascular symptoms. What minimum qualifications you'll need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Education: Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
