In this role, you’ll provide high-quality, one-on-one care to clients in their homes within approximately a 40-mile radius of Grand Rapids. What You’ll Do Travel to client homes or care facilities to complete routine follow-up visits Deliver physical therapy services under the supervision of a licensed Physical Therapist, including implementing restorative care and home exercise programs Complete accurate clinical documentation and progress notes in MatrixCare , ensuring compliance with agency policies and HIPAA standards Collaborate with therapists and the interdisciplinary team to support excellent patient outcomes and smooth discharge planning Educate clients, families, and caregivers on therapy techniques and ongoing care needs Assist with therapy equipment management, program development, and participate in team trainings and goal-setting initiatives Schedule & Location Hours: Full-time and part-time opportunities available Schedule: Monday–Friday (no weekends, holidays, or on-call requirements) Shift: 8:00 AM – 5 PM Coverage Area: Grand Rapids team Qualifications Active Michigan PTA license required 1+ year of PTA experience preferred Experience working with geriatric or medically complex populations strongly preferred Strong ability to work independently while also collaborating as part of a team Excellent time management and organizational skillsA genuine passion for providing compassionate, patient-centered care Generous Paid Time Off (PTO) plan Flexible, customizable benefits package Health insurance and/or FSA options 403(b) retirement plan with employer match Access to free financial advisor consultations Provide m eaningful, one‑on‑one patient care in the home setting Experience autonomy in your day‑to‑day work with strong team support Make a lasting impact on patients’ independence and quality of life Join a collaborative, mission‑driven team dedicated to exceptional care #J-18808-Ljbffr
