Coverage zones are designed to minimize travel and support efficient scheduling. Responsibilities: Evaluate and treat patients in the home health setting. Develop and implement individualized plans of care. Provide therapeutic exercise, manual therapy, and functional training. Educate patients and caregivers. Complete documentation in the EMR system (HelloNote experience is a plus). Qualifications: Active New Jersey Physical Therapist license. Degree from an accredited Physical Therapy program. Reliable transportation for home visits. Strong communication and a patient-centered mindset. New graduates encouraged to apply.
