RN Case Manager OpportunityValley View is looking for RN Case Managers with experience in hospital and Epic preferred!Days/Rotating weekends!We're looking for a PRN Case Manager RN to join our Case Management team! We will hire a Registered Nurse or MSW for hospital discharge planners onsite at Valley View Hospital. Case Management and Epic experience a plus!Previous Case Management experience strongly preferred!General Objectives:Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum.Qualifications:Education:BSN RN preferred.Graduate of an accredited or state board of nursing approved Registered/Professional Nursing programState licensure as a Registered Nurse (RN).1 year of nursing (RN) experienceExperience:Case management experience preferred.Computer experience (word processing, spreadsheet) and familiarity with MeditechWe are interested in every qualified candidate who is eligible to work in the United States.
