Position Details

• Position: Registered Nurse (RN) - Home Health

• Location: Hiawatha, IA

• Employment Type: Local or Travel Contract

• Contract Duration: 13 Weeks

• Schedule: Monday-Friday, 8:00 AM-5:00 PM

• Shift: Day Shift

• Hours: 40 Hours per Week

• Guaranteed Hours: 0 Hours

• Productivity Requirement: 30 Units

• On-Call: Required if requested; on-call needs may change based on staffing and branch requirements.

Job Summary

The Registered Nurse (RN) - Home Health will provide skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care in accordance with established care plans.

The RN will be responsible for conducting comprehensive assessments, completing OASIS documentation, coordinating patient care, and maintaining accurate clinical records using Homecare Homebase (HCHB).

Key Responsibilities

• Deliver skilled nursing care in patients' homes, following established care plans.

• Complete comprehensive patient assessments, including OASIS documentation .

• Administer medications, perform wound care, and provide patient and family education.

• Collaborate with interdisciplinary healthcare teams to coordinate patient care.

• Ensure timely and accurate documentation using Homecare Homebase (HCHB) .

• Manage patient admissions, recertifications, and discharges.

• Participate in on-call rotation as required.

Required Qualifications

• Active RN License - Required.

• 1-2 Years of RN Clinical Experience - Required.

• 2+ Years of Home Health Experience - Required.

• Homecare Homebase (HCHB) Experience - Required.

• OASIS Documentation Experience - Required.

• Current CPR Certification - Required.

• Valid driver's license, reliable transportation, and auto insurance.

Additional Requirements

• Must meet the 30-unit productivity requirement .

• Must be available to work Monday-Friday, 8:00 AM-5:00 PM .

• Must be willing to participate in on-call assignments as requested.

• On-call requirements may change based on staffing and branch needs.

• Guaranteed Hours: 0 Hours.

• Must have experience with Homecare Homebase (HCHB), OASIS, and Home Health nursing .
