Job Details:

Job Title: Home Health LPN

Location: Desoto, TX

Setting: Home Health

Pay Range: 1549.25 - 1626.71

Schedule: 08:00 - 17:00

Contract Length: 13 weeks

Candidate Type: Traveler or Local

Requirements: 1 year of Medicare certified Home Health experience.

Responsibilities:

Provide skilled nursing care to patients in a home health setting.
Administer medications and treatments as prescribed by physicians.
Monitor patient health status and update care plans accordingly.
Educate patients and family members on health management.
Maintain accurate documentation of patient care activities.
