About the Position

Job Title: Registered Nurse I Profession: Nursing Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty. License: Must have a valid nursing license in hand. Certifications: CPR, BLS Must-Have: Recent specialty experience in home health or hospice. Proficiency in HomeCare HomeBase EMR and Oasis. Reliable transportation, current driver's license, and agency-required liability insurance. Description: This position coordinates and directs the provision of home nursing care according to physician's orders. Care is delivered based on agency policies and procedures. Responsibilities include the competent application of the nursing process. The nurse is responsible to the Clinical Manager. Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses or Licensed Vocational Nurses is part of this role. Home visits are required. Periodic office meetings may also be necessary. The average number of patients per day is 5-8. Productivity is measured by points assigned to various visits and activities, totaling 30 points per week. Types of cases include Start of Care, Wound Care, and Follow-up Visits. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation may affect patient care and follow-up. Clinicians must accept all patients on their schedules, regardless of distance. There are no guaranteed hours with this contract. The clinician must achieve weekly productivity points of 30, which equates to 40 hours of work. Travel may be required up to 75 miles from the home clinic. Local candidates are accepted at the same rate.
