Role Overview

As a Home Health RN, you will serve as the clinical lead for your patients, overseeing the care plan, coordinating with various disciplines, and driving outcomes from admission to discharge.

You will integrate advanced assessment skills, care planning, and patient education to minimize hospitalizations, enhance recovery, and promote long-term independence at home.

Your Responsibilities
  • Perform thorough in-home assessments to evaluate clinical status, risks, and care objectives.
  • Develop and oversee personalized care plans in partnership with physicians and care teams.
  • Provide skilled nursing care, including medication management, treatments, and interventions.
  • Act as the case manager, coordinating care among therapists, aides, and physicians.
  • Monitor patient progress and make clinical decisions based on evolving conditions.
  • Educate patients and families to facilitate disease management and independence.
  • Coordinate care transitions and discharge planning to minimize readmissions.
  • Maintain timely and accurate documentation in the electronic medical record (EMR).
  • Supervise and support Home Health Aides as necessary.
  • Participate in on-call rotation as required.
Qualifications and Requirements
  • Current RN license (or eligibility for compact license) and CPR certification.
  • Graduate from an accredited nursing program.
  • At least one (1) year of RN experience (preferably in Home Health, Med-Surg, ICU, or Acute Care).
  • Strong clinical judgment and the ability to think independently.
  • Excellent communication skills with a patient-centered approach.
  • Capability to manage a caseload and prioritize tasks effectively.
  • Proficiency in using EMR systems.
  • Valid driver's license and dependable transportation.
