This position serves a dual role, combining contract management and oversight responsibilities for the Department's Memory Disorder Clinics (MDC), Florida Brain Bank, and related Alzheimer's disease and related dementia (ADRD) initiatives with direct care navigation services for individuals living with Alzheimer's disease and related dementia (ADRD) and their caregivers. The position requires strong knowledge of ADRD services, resources, contract management, relationship building, and person- and family-centered care.

Pay: Salary will be commensurate with experience and qualifications, ranging from $53,500.08, annually.

Duties and Responsibilities:

Contract Management and Program Oversight:

• Serve as Contract Manager for the Department's Memory Disorder Clinics (MDC), Florida Brain Bank, and Alzheimer's disease and related dementia (ADRD) projects.
• Monitor and administer assigned contracts to ensure compliance with Section 287.058(1)(e), Florida Statutes, and all applicable federal and state laws, rules, policies, procedures, and Department requirements.
• Ensure that contract deliverables are quantifiable, measurable, and verifiable and that deliverables are received, reviewed, and accepted in writing prior to payment, as required.
• Monitor contractor performance, provide technical assistance as appropriate, and ensure that contract activities and deliverables are completed in accordance with established timelines and requirements.
• Maintain complete and accurate contract files in accordance with Department policies, including required documentation in SharePoint and other designated systems.
• Collaborate with Contract Administration, Finance and Accounting, the Monitoring Unit, and other Department staff to ensure all contract requirements, deadlines, amendments, payments, and reporting obligations are met.
• Process invoices, requests for advancements, and other contract-related documentation.
• Track surplus and deficit reports, eCIRTs, and other relevant contract and program data.
• Maintain continuing professional education and training in the contracting process as deemed appropriate by the Department, including an annual review of the Department of Financial Services' Contract Manual and any applicable updates.
• Maintain up-to-date knowledge of federal and state laws, rules, policies, and procedures pertaining to assigned programs and contract management responsibilities.
• Maintain current knowledge of legislative, policy, research, and programmatic developments related to Alzheimer's disease and related dementias, including issues affecting the Memory Disorder Clinics, Florida Brain Bank, Alzheimer's Disease Advisory Committee (ADAC), Dementia Care and Cure Initiative (DCCI), and other Department initiatives.
• Assist with related Department initiatives and review and edit materials for dissemination both within and outside the Department.
• Prepare for and host quarterly meetings with MDC clinics.

Care Navigation and Direct Client Support:

• Provide direct education, guidance, and ongoing support to individuals living with Alzheimer's disease and related dementias (ADRD), their families, and caregivers to assist them in obtaining appropriate medical care, community-based services, and other supports necessary to promote optimal care and quality of life.
• Develop and maintain trusted, ongoing relationships with caregiving families and provide case management-style care navigation services.
• Consultations may include the development of individualized care plans, identification of available resources, follow-up with families, problem-solving, and coordination with appropriate providers and organizations.
• Provide timely and compassionate support to families by phone, email, and other appropriate methods.
• Practice empathetic listening, validation techniques, and person- and family-centered approaches while maintaining confidentiality and accurate documentation of all interactions.
• Maintain high-level knowledge of ADRD resources and services within the assigned Planning and Service Area, as well as statewide and national programs and services available to individuals living with ADRD and their caregivers.
• Support and provide information regarding broader Department services that impact family caregiving, including the Elder Helpline, Memory Disorder Clinics, and other available programs and resources.
• Remain knowledgeable and current regarding emerging ADRD research findings, evidence-based practices, national and state initiatives, and rules and regulations affecting ADRD care.
• Continuously enhance professional knowledge, skills, and abilities through education, training, seminars, workshops, and review of relevant policies and procedures.
• Serve as a resource and consultant to Department staff and external partners regarding ADRD services and resources within the assigned region.
• Engage regularly with the Department's Dementia Director, Professional Review Team, and other Care Navigators to promote collaboration, consistency, and high-quality service delivery.
• Participate in pilot programs and special projects involving collaborative efforts to improve the assessment, coordination, and delivery of services for individuals living with ADRD and their caregivers.

Relationship Building and Network Development:

• Develop and maintain collaborative relationships with key partners, organizations, and stakeholders serving individuals living with ADRD and their families.
• Work collaboratively with the Dementia Care and Cure Initiative (DCCI) Task Forces and other local, state, and national partners to strengthen the network of available ADRD resources and supports.
• Collaborate with organizations and entities including, but not limited to, the Agency for Health Care Administration, Department of Health, Department of Children and Families, Florida's Memory Disorder Clinics, Florida Brain Bank, Alzheimer's Association, AARP, Aging and Disability Resource Centers, and other community partners.
• Support the Department's statewide age-friendly and dementia-capable efforts by participating in meetings, committees, presentations, conferences, and other outreach opportunities as assigned.
• Assist in educating communities and partners regarding the Department's ADRD initiatives, including the Memory Disorder Clinics, Florida Brain Bank, DCCI, ADAC, and other related efforts.
• Develop and maintain relationships with existing and potential partners by attending council and committee meetings, participating in speaking engagements, and sharing information and resources with individuals and organizations interested in active aging, aging in place, dementia-capable communities, and programs that support the mission and vision of the Department.

Strategic Planning, Continuous Improvement, and Performance:

• Proactively identify gaps in ADRD services, resources, and programs within the assigned region and statewide.
• Work collaboratively with Department leadership, internal staff, and external stakeholders to identify and address short- and long-term needs and opportunities for improvement.
• Provide feedback and information gained through direct interaction with families and community partners to help inform future program development, service delivery improvements, policy considerations, and Department initiatives.
• Throughout all assigned responsibilities, meet established measurable outcomes, timelines, benchmarks, and performance expectations.
• Assist in tracking individual and team progress toward established goals and support adherence to best practices, policies, and procedures.
• Ensure a high level of responsiveness, follow-through, record-keeping, communication, and problem-solving across both contract management and care navigation responsibilities.

This position combines administrative oversight, direct client support, and cross-functional collaboration to promote high-quality, client-centered services and strengthen Florida's ADRD system of care. This position reports to the Florida Alzheimer's Center of Excellence Manager and Dementia Director. The incumbent will be based out of Department Headquarters in Tallahassee and may work remotely within the assigned territory, as authorized. The position will engage regularly with the Dementia Director, Professional Review Team, Care Navigator team, contract providers, and other Department staff and external stakeholders.
