Behavioral Health Nurse - Rural Counties

NorthKey Community Care is seeking a full-time Behavioral Health Nurse RN to engage and assist in the care of behavioral health clients in an outpatient setting. This position will split a schedule between two offices (Falmouth and Williamstown). Work hours are generally Monday-Friday from 8:30 a.m. - 5:00 p.m.

QUALIFICATIONS
- An active RN license in Kentucky is required.
- Relevant knowledge, training, and experience.
- Excellent written and verbal communication skills.
- Psych nursing is preferred but not required.
JOB DUTIES
- Assist physicians and nurse practitioners in the care of patients as part of medication management as well as in the development of service plans based upon assessment data.
- Assist physicians and nurse practitioners conducting hospital discharge clinics for medication assessments and prescribing as indicated by patients' presenting issues.
- Conduct routine screenings for the need for additional therapeutic and physical health services.
- Provide consultation to other staff as indicated.
- Complete quality documentation of services provided, including service plans, progress notes, medication informed consent, treatment plan reviews, and other required information to assure compliance with organizational policies and third-party compliance standards.
- Ensure patients are seen routinely prior to providing medication refills.
- Adhere to appropriate standards of care.
- Assist physicians and nurse practitioners in monitoring individuals on caseload for changes or decline in mental status or medical conditions.
- Collaborate with other staff members to promote quality patient care.
- Participate in interdisciplinary team communications.
- Handle medication orders and medications by following policies and relevant state regulations and quality standards.
- Ensure appropriate communication with other internal and external professionals for effective delivery of quality patient care.
