Req Number: DIR‑26‑00039 Location: IBHS Mercer – PACT Team I and II (Trenton, NJ) Full‑Time/Part‑Time: Full‑Time Number of openings: 1 Category: Direct Care – Clinical Job Summary Under the supervision of the Team Leader, the Clinician will be a member of the team responsible for the coordination and delivery of professional behavioral health services designed to meet the wellness and recovery needs of the consumer. These services may include personal care, life skills and resource development, assessments and counseling provided directly to the consumer or through coordinated efforts of the PACT Team. Essential Functions Conduct assessments consistent with the identified clinical specialty upon intake and/or upon request by the consumer and/or other PACT team members. Contribute, under the supervision of the Team Leader, in resource development, supervision, training and coordination of PACT consumers and their wellness/recovery activities with respect to the delivery of their identified clinical specialty. Provide individual, group and family therapy services; conduct education and counseling programs addressing the needs of the consumer and his/her family; provide on‑call and emergency response services. Maintain accurate and current records on all consumers, including charts, progress notes, service/recovery plans and other reporting mechanisms. Participate in intake screening, service planning and discharge review processes; coordinate and advocate with community service providers for ongoing, emergency and other health care for PACT consumers. Serve as a member of the PACT team providing ongoing case management and/or specialized services consistent with training/licensing/certification. Establish and maintain a therapeutic relationship with consumers in hospital, community, and with agencies and family as appropriate. Participate in development of individualized treatment plans, oversee resource development, provide PACT team training, and serve as backup support to members of the PACT team. Provide on‑call services 24/7 and ensure accurate documentation of services. Participate in daily planning and coordination meetings. Other Duties Provide representation on behalf of the agency, the PACT team, and/or the consumer at community‑based medical, treatment planning, and service coordination meetings. Contribute to interdisciplinary in‑service training and case conferences. Participate in the quality improvement process and specific quality initiatives for the program. Perform other duties as deemed necessary by the Team Leader. Minimum Qualifications Master’s Degree in Behavioral Science/Human Services (e.g., Occupational Therapy, Psychology, Social Work, Special Education, Rehabilitation, Family Counseling) from an accredited college or university. Two years’ experience in the delivery of community‑based social services to individuals with serious mental illness. Valid driver’s license and ability to travel independently to consumers’ residences, work sites, and hospital settings. Ability to be on‑call and provide emergency response. Licenses/Certifications Needed Certification/licensure (LPC, LAC, etc.) is preferred but not required. Knowledge/Skills/Abilities Bi‑lingual capacity (Spanish/English) is desirable. Employment Details Exempt/Non‑Exempt: Non‑Exempt Weekly work hours: 40 Weekly pay: $55,000 annually Schedule: Monday–Friday 8:00 AM–4:00 PM with evening and weekend availability; on‑call required.
