Our healthcare partner is looking for a skilled Physical Therapist to join their team and make a meaningful impact in the Bala Cynwyd, PA area. As a Physical Therapist in this role, you will be responsible for delivering high-quality care to patients in a home health setting. Your expertise will help patients regain their independence and improve their quality of life. You will work closely with a supportive team to ensure the best outcomes for your patients.
- Provide physical therapy services to patients in their homes
- Conduct start of care assessments and revisits
- Utilize EPIC and OASIS for documentation and care planning
- Collaborate with other healthcare professionals to develop and implement individualized care plans
- Need 1 year experience in setting
- Recent and relevant home health experience required
