Telehealth Substance Use Disorder (SUD) Social Worker (LCSW / LMSW) At $75,000 to $85,000 and fully remote within New York, this role is for a social worker who already has real experience handling substance use and wants to do the work without the commute. As a Telehealth Substance Use Disorder Social Worker at Nao Medical, you will deliver virtual assessments, counseling, and care coordination for patients living with substance use disorders, using evidence-based, trauma-informed practices and connecting them to treatment, medication, and recovery support. You will coordinate closely with our CASAC counselors, peer advocates, and prescribing providers, with AI copilots handling the documentation and a connected care team a message away. Conduct psychosocial and substance use assessments and complete OASAS LOCADTR level of care determinations. Counsel and intervene. Provide individual and group counseling using Motivational Interviewing, CBT, and other evidence-based modalities. Support medication-assisted treatment. Coordinate care for patients on MAT alongside prescribing providers. Coordinate and connect. Build recovery-focused care plans and link patients to treatment, harm reduction, housing, and community resources. Partner with the SUD team. Work closely with CASAC counselors and peer advocates to keep patients engaged in care. Document to standard. Keep accurate, timely records that meet OASAS and New York State requirements, with AI copilots assisting.
