Public Health Nurse II

Description Provides professional services for the community in the delivery of health promotion/disease prevention activities and case management primarily focused in Family Health. Primary Duties & Responsibilities: • Provides Professional Public Health services to individuals and families in the community.
• Participates in health promotion clinics and related activities as assigned. • May conduct disease outbreak follow-up and control as assigned. • Participates in public health emergency preparedness planning. • Completes assessments for eligibility programs offered through state waivers • Provides care coordination for various health plans as contracted for senior clients • Ability to provide immunizations to all ages when necessary.

• Ability to utilize effective teaching methods in relating to clients, families, groups and staff.
• Computer experience required. Minimum Qualifications:
• Current licensure as a Registered Nurse in the State of Minnesota.
• Subject to criminal history background check.
