Heartland Hospice is seeking a Nurse Practitioner to deliver advanced, patient‑centered care across hospice and palliative services. You will perform comprehensive patient assessments, manage visits including Hospice F2F for certification, and collaborate with IDG/IDT to develop individualized care plans. You will prescribe medications within state and regulatory scopes, coordinate care across settings, and maintain compliant documentation while supporting families through serious illness. #J-18808-Ljbffr Gentiva
