Medicaid Care Coordinator - Health Home Specialist
Postgraduate Center for Mental Health
Post Graduate Center for Mental Health is seeking a Care Coordinator to support NYC Medicaid beneficiaries with chronic health and/or behavioral health needs. You will work within a Health Home model, coordinating care, performing outreach between visits, and advocating with providers and community resources.
The role emphasizes engagement, care plan development, access facilitation, and timely documentation. Strong communication and cultural competence are essential for success.
