Paramount Premier Homecare LLC is looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team in Bowmansville, NY. Responsibilities:

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Assess and document patient health status, progress, and changes in condition
• Collaborate with RNs, physicians, and care team members to implement individualized care plans
• Educate patients and family members on care procedures, medications, and health management
• Perform skilled nursing tasks in a home care setting in accordance with physician orders
• Maintain accurate and timely clinical documentation and patient records
• Ensure patient safety and comfort while promoting independence and dignity

Requirements:

• Active Licensed Practical Nurse (LPN) license in the state of New York
• Previous home care, long-term care, or clinical nursing experience preferred
• Strong knowledge of nursing procedures, infection control, and patient safety protocols
• Excellent communication and interpersonal skills for patient and family interaction
• Ability to work independently and manage time effectively in a home care environment
• Valid driver's license and reliable transportation required
• CPR/BLS certification preferred
