RN – Home Health Northeast Philadelphia & Lower Bucks County $36 - $41 per hour – Flexible scheduling – Full‑time, part‑time, PRN Position Overview As a Home Health RN, you will provide skilled nursing care to patients in their homes while collaborating with the interdisciplinary team to ensure high‑quality, individualized care. Responsibilities Deliver skilled nursing care in accordance with the established plan of care Monitor and assess patient conditions during home visits or shifts Communicate changes in patient status to the Clinical Supervisor Provide patient and caregiver education regarding medications, treatments, and disease management Accurately document patient care and clinical observations Coordinate care with other healthcare team members as appropriate Maintain compliance with agency and state guidelines Schedule Options Day, evening, and overnight shifts available Weekday and weekend opportunities Full‑time, part‑time, PRN, per diem Choose your own hours based on availability Compensation $36.00 – $41.00 per hour (pay rate based on experience, case complexity, and assignment type) Opportunities for overtime and holiday scheduling are available.
