Care Coordinator LC JAIL
Care Coordinators are part of the Addiction Response Team and are responsible for working in collaboration with local providers and community agencies to ensure individuals with substance use are successfully connected with resources to aid in their long-term health, treatment and recovery goals. Care Coordinators will aid individuals in meeting their goals toward ongoing recovery through education, outreach, connectivity to Medication Assisted Treatment (MAT) and other treatment and recovery services. Care Coordinators will report to Care Coordination Supervisor.
Role Summary:
This position works with individuals who are incarcerated and engaged in Medication-Assisted Treatment to provide assistance and resources for mental health, substance treatment and recovery, housing stability, food security, legal resources, and other needs. This position is focused on assisting these individuals with re-entry into the community. This position works directly with the Larimer County Jail and CorrHealth, the medical provider located in the Larimer County Jail.
Knowledge / Skills / Abilities
- Ability to effectively engage with different populations including People of Color, LGBTQIA+, Disability, Veteran, Low/No Income and/or experiencing homelessness, People with criminal justice involvement, and people who use drugs, people with mental health conditions, people with complex medical conditions and more.
- Skills to communicate and interact appropriately and respectfully across multi-cultural differences.
- Strong written and verbal communication skills (both over the phone and in person)
- Strong decision making, critical thinking, and problem-solving abilities.
- Experience with persons who have a substance use disorder and knowledge of medication assisted treatment.
- Experience partnering with local and regional health and human services resources.
- Ability to recognize different pathways to recovery and showing empathy toward others with substance use disorders and their journey.
- Ability to work independently and in a busy office environment that may contain interruptions to work due to walk-ins, member needs, supervisor requests, phone conversations, etc.
- Able to manage stressful situations while remaining calm and non-reactive (i.e. working with members who may have severe and persistent mental health issues or may be under the influence of a substance).
- Ability to adjust priorities quickly as circumstances and needs dictate for member success.
- Ability to effectively engage and build rapport in a variety of medical, behavioral health, justice involved, substance use, and homeless situations.
- Strict Confidentiality required.
- Basic Computer Skills preferred for navigation through multiple Electronic Medical Record Systems and data collection tools as determined by organizational needs.
- Reliable transportation
- Bi-lingual (Spanish) preferred
- Valid Driver License
Education or Formal Training
- HS Graduate or GED required
- Certified Addiction Technician Preferred
Licensure/Certifications
- Valid Colorado Driver License and Insurance required

Job Responsibilities:
- Enrollment of persons using appropriate assessment tools and completion of GPRA Survey at enrollment and then at 6 month follow up
- Support persons enrolled in services to continue engagement with both medical and behavioral health treatment plans and assist persons in scheduling appointments, follow-up care, referrals, medication refills etc.
- Provide appropriate health needs and clinical/non-clinical assessment identify risk factors and develop health plan and support with the integration of treatment and recovery services including Medication Assisted Treatment.
- Enter all documentation, contacts, and assessments into required databases within 2 business days.
- Participate in telephone contacts, court visits, office/hospital visits, community visits and other means of connecting with members to reach identified goals.
- Perform health needs assessments and identify risk factors to develop health care plan
- Assist members in scheduling appointments, follow-up care, referrals, medication refills, etc.
- Attend member appointments with providers and community resources as needed.
- Work in collaboration, communication, and coordination among all responsible parties of an individual ‘s multidisciplinary team to support achieving goals and reduce duplication of services.
- Optimize member and family self-sufficiency through education, community resources, and support.
- Understand, communicate, and facilitate on behalf of patients with regard to complaints, grievances, and appeal processes.
- Regular Case Reviews with a member of the Addiction Response Team Leadership.
- Completion of Addiction Technician Certification (CAT) and adhere to supervision as required.
- Transport members across the region as needed in an NCHA company vehicle.

Other Duties as Assigned:
- Provide assistance as designated by the ART leadership to support the overall goals of NCHA.
- Other duties as needed to meet demands of the organization, which may include flexing or changing job locations.
- Provide input into development of procedures and workflows, and be accountable for adhering to them.
- Support of the mission and vision of NCHA
- Compliance with NCHA programs and procedures overall
- Meeting and training attendance.
Participation in ongoing performance improvement activities.
