Responsibilities Provide psychosocial support and counseling to hospice and palliative care patients and their families. Serve as the social work member of the interdisciplinary team and provide counseling and case management services. Complete 20‑25 visits per week and document all work in the electronic medical record. Formulate a social work plan of care, including goals and interventions, and make community referrals. Develop palliative social work plan of care and recommend based on patient/family needs. Comply with all required documentation policies and procedures. Document goals and interventions in the plan of care with specific end dates and outcomes. Provide transitional planning with community linkages and screening for hospice eligibility. Discharge planning begins when no additional palliative care case management is required. Qualifications Graduate from an accredited School of Social Work. Minimum of 3 years of social work experience in a healthcare setting. Preferred experience in community health, hospice, or palliative care. Must hold an LMSW license. Must have a car and a valid driver’s license.
