What You’ll Do Complete initial home health assessments and full physical evaluations Plan, deliver, and evaluate skilled nursing care using the nursing process Ensure accurate OASIS scoring through comprehensive assessments Develop patient‑centered care plans with families included in decision‑making Manage your own caseload and play a key role in helping patients regain strength and independence What We’re Looking For Required: RN from an accredited nursing program (Associate degree minimum) 1+ year of recent acute care experience (med/surg or acute rehab preferred) Strong clinical judgment and ability to work independently CPR certification Clean criminal and driving record (mileage reimbursed) Preferred: BSN Community Health/Education background Additional clinical certifications (IV, Chemo, Ostomy, etc.) Familiarity with OASIS and reimbursement drivers If you’re seeking work-life balance, professional support, and meaningful patient connections , this is an outstanding opportunity to join one of Southern NH’s most respected healthcare organizations. #J-18808-Ljbffr
