Job Title Physical Therapist Location Arizona | Outpatient Schedule / Job Type Contract | Days | 5 days / 8 hours per day Key Responsibilities Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities Delegate and direct all discipline-specific therapy services for assigned patients Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations Adhere to discipline-specific State Practice Act and all regulatory agency requirements, maintaining professional licensure in good standing Required Qualifications Active Physical Therapist license in good standing in the state of Arizona Completion of all state-mandated continuing education requirements Ability to perform in an ethical manner in accordance with discipline-specific professional code of conduct Strong clinical reasoning skills with the ability to develop appropriate treatment plans and goals Preferred Experience / Skills Experience in orthopedic (ORTHO) outpatient physical therapy Experience with student supervision and clinical education Familiarity with performance improvement methodologies and organizational quality initiatives Why Join Us Long-term contract opportunity running through June 2027, offering stability and continuity of care in an outpatient orthopedic setting Supportive environment that encourages professional development, continuing education, and involvement in professional organizations Concentric Healthcare Staffing Job ID 776176.
