Associate Patient Care Coordinator

Patient Care Coordinator (RN) – HospiceLocation: Leawood, KS Job Type: Full-Time | Salaried | ExemptAt Legacy Hospice, we believe in providing exceptional end-of-life care with dignity, compassion, and respect. We are seeking a skilled and compassionate Patient Care Coordinator (PCC) to lead our clinical team and ensure our patients receive the highest quality of care in alignment with our mission and values.What You'll DoAs a Patient Care Coordinator, you will oversee the day-to-day coordination of patient care, supervise the interdisciplinary clinical team, and support the Branch Administrator in managing operations and ensuring compliance. You'll play a key leadership role in maintaining clinical excellence and fostering a culture of compassion.Supervise and support hospice clinical staff in providing exceptional patient careEnsure compliance with state, federal, and accreditation regulationsConduct and facilitate interdisciplinary team (IDT) meetingsParticipate in staff hiring, training, and performance evaluationsOversee admission, discharge, and transfer processes for patientsEnsure timely documentation, service coordination, and regulatory reportingSupport quality assurance and performance improvement (QAPI) activitiesMaintain readiness for surveys and regulatory visitsProvide direct patient care and RN case management, as neededCollaborate with community providers, ancillary services, and internal departmentsServe as acting Branch Administrator when neededQualificationsRegistered Nurse (RN) licensed in the state of employmentGraduate of an accredited school of nursing; BSN or CHPN preferredMinimum 1 year of hospice, home health, or related nursing experienceExperience supervising clinical and paraprofessional staffExcellent leadership, communication, and time management skillsActive BLS (CPR) certificationValid driver's license, auto insurance, and reliable transportationSelf-directed and capable of managing priorities independentlyPlease note: Legacy Hospice is a drug-free workplace.
