Description

• Conduct thorough evaluations in either inpatient or outpatient settings and create customized treatment plans for each patient
• Implement interventions targeting mobility, strength, balance, and overall function
• Collaborate closely with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation experts to achieve optimal patient outcomes
• Guide patients and their families through therapeutic goals, discharge plans, and home exercises
• Document patient evaluations, treatments, and progress according to established facility protocols
• Participate in care team discussions and interprofessional meetings to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) from an accredited institution or an equivalent qualification

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Prior experience in a clinical setting is advantageous
