Location: Eureka, CA Start: 09/21/2026 Duration: 13 weeks Shift: Days | 58s | 8:00 AM4:30 PM
Requirements: 2+ years acute care hospital Case Management experience Case Management experience must be current within the last year Care coordination & discharge planning experience required Experience with HMOs, IPAs, advance directives, POLST & protective services Epic experience required Active CA RN License Travel experience preferred; first-time travelers may be considered with approval
Additional: Patient caseload up to 25 Patient ratio approximately 1:20 Every other weekend required No call May float based on department needs Locals accepted Free parking on first day
