Looking for a Full Time LPN to serve Centerville, Kettering, and Beavercreek areas.

Pay Range: $23.00-33.00/hour based upon experience

Position Summary:

The Licensed Practical Nurse is responsible for assessing, planning, implementing, and evaluating the nursing care for a caseload of clients from admission to discharge. Coordinates and supervises the nursing care delivered by the home health aides working under the Administrator of Health at Homes supervision. Assumes Case Management Responsibilities as assigned.

LPN Job duties:

• Coordinates processes to support clients medication administration based on individual need.
• Assesses for significant changes in a clients condition and updates care plan accordingly.
• LPN as assigned, supervises the Home Health Aide in the home
• Initiates and completes all necessary documentation and reporting necessary to support services and charges.
• Communicates all order and status changes to the physician.
• Maintains a safe environment for clients and employees while exhibiting a positive attitude towards all customers.
• Maintains individual care plans based upon comprehensive assessment.
• Attends in services, training, staff meetings, care plan conferences, and seminars as scheduled.
• Responsible for completing nursing notes according to Medicare/Medicaid guidelines and Health at Home charting guidelines.

Qualifications:

• Valid drivers license and safe driving record
• Licensed Practical Nurse from an accredited school of nursing with a current Ohio license.
• Two years of nursing experience one year must be home health care experience preferred.
