Care Manager RN - Per Diem

Seeking a Care Manager RN to lead patient-centered care coordination across the continuum. Responsibilities include developing safe discharge plans in collaboration with patients, caregivers, and healthcare teams, optimizing patient flow, reducing length of stay, and limiting readmissions through innovative, evidence-based practices. Key duties involve assessing physical, psychosocial, and economic needs, documenting interventions, communicating with outpatient providers, and ensuring compliance with policies and regulations. Qualifications include an Associate's Degree in Nursing, current RN license, and at least one year of hospital or acute care experience.
