Physical Therapist - Coordinator

Easterseals is seeking an experienced and motivated School-Based Physical Therapist Coordinator to lead and support physical therapy services in a school base setting. This role is ideal for a licensed Physical Therapist who thrives in a leadership role and is passionate about helping children succeed in a school-based setting. What You’ll Do Conduct initial and ongoing evaluations, maintaining accurate documentation and progress notes. Develop individualized, goal-driven treatment plans and reassess students according to established timelines. Collaborate with interdisciplinary teams to optimize scheduling and maximize therapeutic outcomes. Oversee scheduling for evaluations, referrals, and screenings. What You Bring Active Illinois Physical Therapist license. Minimum of 2 years of experience in a school-based physical therapy setting. Current Child CPR and First Aid certification (or willingness to obtain). Prior supervisory or leadership experience preferred. Strong organizational, communication, and collaboration skills.
