Full Time | Day Shift

Pay: $79,710-$89,674
based on experience

United Methodist Communities in Camden County, a 5-Star full-service senior living community that is a model in person-centered care community, located in beautiful Collingswood, NJ is seeking a Care Coordinator.

Care Coordinator Requirements:

• Proficient in Microsoft Office applications
• 1 year of experience in case management marketing
  • 2 years of experience in case management marketing preferred
• One of the following licenses or a bachelor's degree in a related field from an accredited college or university:
  • LPN
  • RN
  • LCSW

What You'll Do:

• Gather and evaluate referral information, review submitted materials, and/or assess potential residents for admission
• Make admission decisions and/or collaborate with nursing staff on healthcare admission determinations
• Build and maintain a referral network, including conducting onsite visits as needed
• Facilitate the admission process into healthcare, ensuring all required paperwork is completed and processed in a timely manner
• Verify Medicare coverage and/or other insurance benefits
• Participate in care conferences and other required meetings
• Follow up with residents post-discharge to ensure well-being; complete medication reconciliation and psychosocial assessments
• Support post-acute discharge care in the home setting
• Ensure departmental reports are completed accurately and on time
