Role Overview RiverWoods Exeter is seeking a full‑time 32‑hour per week RN/LPN to provide direct resident care in a team‑focused environment. The position includes a weekend component, requiring work on every other weekend. Responsibilities Perform professional duties in accordance with facility policies, state regulations, and professional standards. Collaborate with interdisciplinary team members to formulate and update individualized resident care plans, and participate in Resident Care Plan Meetings. Institute care protocols for falls prevention, safety, elimination, behavior management, weight management, skin integrity, bowel/bladder management, infection control, and pain assessment and management. Assess, document, and report changes in residents’ physical or mental status to the primary care physician and treatment team. Provide leadership and direction for direct caregivers, prepare daily assignments, delegate workload, and supervise direct care. Establish positive working relationships with team members, residents, and family members to facilitate open communication. Obtain, administer, and document medications and treatments following guidelines and policies; inventory and maintain narcotic lock system. Support, create, and maintain a positive culture for team members and residents. Qualifications Minimum nursing degree or diploma from an accredited institution. Experience in long‑term care and specialized knowledge of nursing care of the elderly and rehabilitative nursing preferred. Registered nursing (RN or LPN) license in the State of New Hampshire. Certified in intravenous administration preferred. Must maintain CEUs annually; CPR required. #J-18808-Ljbffr
