Alpha Home Health and Hospice is a Medicare-certified, 5-Star rated agency seeking an experienced and caring Full‑time Bachelor’s Level Medical Social Worker (BSW) for our Hospice division in the greater Snohomish County area. Assess the psychosocial status of patients related to patient’s illness and progression Develop and provide multiple community and governmental resources to support a patient in their home environment Provide assessment in patient’s residence, carry out social evaluations, and plan interventions Participate in the development of the individualized plan of care Assist physician in understanding significant social and emotional factors Participate in quality assessment and performance improvement team Serve as liaison between patients and community agencies Maintain and develop contracts with public and private agencies Assist with bereavement coordination Qualifications A graduate of an accredited bachelor’s or master’s program in social work Minimum of one year’s experience in health care is required Experience with home health or hospice is strongly preferred Possesses and maintains current BLS Certification A licensed driver with an insured automobile Requires weekend rotation: Saturday and Sunday 8‑5 or 9‑6.
