Brain Injury Case Manager & Social Worker (Home-Based)

REM Community Services, a part of Sevita, seeks a Social Worker to evaluate and treat clients in home and community settings under physician orders. You will develop and adjust treatment plans, gather data, and document progress. You will conduct one-to-one counseling, educate clients and caregivers about brain injury, attend team conferences, and ensure timely billing and reporting.
