This role involves advocacy, service linkage, and monitoring to ensure continuity of care and prevent service gaps.**Persons with lived experience in mental illness, developmental disabilities, or substance use disorders are encouraged to apply.**-**Key Responsibilities*** Develop and implement person-centered service plans* Link individuals to medical, social, educational, and therapeutic resources* Coordinate and monitor services to achieve successful outcomes* Provide advocacy and support to reduce barriers to care* Participate in emergency on-call rotation-**Qualifications*** Bachelor's degree in a human services field* Minimum one year of experience in mental health* Certification as QMHP, QIDP, and/or CMHP, or Michigan licensure (limited or full)* Strong organizational, communication, and problem-solving skills* Ability to work flexible hours and travel within service area* Valid Michigan driver's license, vehicle insurance, and satisfactory driving record-**About Wellvance**Wellvance provides state-funded mental health services to residents of Iosco, Ogemaw, and Oscoda counties.
