Treatment Adherence Specialist: HIV Care & Outreach

Community Healthcare Network

Community Healthcare Network (CHN) is seeking a dedicated staff member to enroll and assess clients in the Treatment Adherence Program. You will maintain detailed records and keep the primary care provider informed about progress. This role emphasizes outreach, education, and coordination with medical staff to improve access to care.
