Responsibilities Providing initial and ongoing assessment related to physical and functional status, home safety, as well as cognitive, psychological and perceptual function to determine baseline and appropriate, achievable goals. Developing and revising the plan of care in collaboration with patient/family/caregiver and other care-team members. Planning, evaluation, and treatment implementation of assigned caseload. Preparing clinical progress notes. Supervising and guiding home health aides as appropriate. Evaluating outcomes and planning discharge. Educating patients and/or families about proper safe use and recommending methods for installation of adaptive equipment. Territory North King County (Ballard, Fremont, UW, Greenlake and up through Richmond Beach) Qualifications Must have OT license registered in the State of Washington. Current driver's license and car insurance. Home Health experience highly preferred but not required. Able to maintain strict confidentiality with regard to protected and sensitive information. Must be able to solve problems independently and confidently by applying analytical and logical thinking. Prioritize workload to meet productivity standards and applicable deadlines. Demonstrate flexible and efficient time management techniques. Able to use standard office equipment and software proficiently.
