Primary Care (Family Medicine and Internal Medicine)

AtlantiCare's primary care practices are taking meaningful steps toward the "patient-centered medical home" model through a commitment to same- or next-day urgent appointments, electronic medical records and collaboration with an AtlantiCare care manager for complex patients. Additionally, the practices are supported by AtlantiCare hospitalists, who care for patients admitted to our two medical centers.
