Full Time Evenings, Every Other Weekend

1 year experience as LPN required.

• New York LPN Licensure
• BLS certification (AHA or RQI required)

• Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and other team members.
• Contribute to creating an age-appropriate and culturally relevant care plan, involving residents/families and healthcare teams.
• Recommend revisions to the plan of care based on evaluations of resident outcomes and ensure all care aspects are addressed.
• Participate in mentoring or orienting staff while considering safety, cost, and planning in the delivery of care.
• Maintain current knowledge and competency in Nursing Practice.
• Demonstrate leadership within professional practice under the RN's direction.
