Job Summary: Tryfacta is seeking an LPN - LTC for our client in Berlin, NH 03570 . This is a 13-week contract assignment. Position Title: LPN - LTC Location: Berlin, NH 03570 Duration: 13 weeks (Possible extension) Work Schedule: 8/16 hrs. (Day, Evening and Night) Guaranteed Hours: 40/wk. Pay rate: Local - $45/hr. and Traveler: $50/hr. ( $2000/wk. ) Overtime: Yes (including potentially one extra shift.) Total Beds: 100 Current Census: 96 Shift: Primarily 2nd and 3rd shifts Responsibilities for this position include, but are not limited to: Provide direct nursing care to residents in a long-term care setting. Administer medications and treatments according to physician orders and facility protocols. Manage approximately 15 patients per medication pass , with Medication Aides assisting with certain medications. Assess residents and monitor for changes in physical, emotional, and clinical condition. Report significant changes in resident condition to the appropriate healthcare provider. Provide patient education regarding medications, treatments, health conditions, and care plans. Provide guidance, education, and support to LNAs/Nurse Aides. Collaborate with Medication Aides, Nurse Aides, RNs, physicians, and other members of the healthcare team. Assist with development and implementation of individualized care plans. Maintain accurate and timely patient documentation using PointClickCare (PCC) . Follow infection-control, safety, and facility policies. Promote resident dignity, comfort, safety, and independence. Support quality patient care while maintaining a strong bedside presence. Work effectively within established staffing ratios and assist with unit needs as required. To be considered for this position, you should have: [Skills, Education, or Experience] Active LPN/LVN license. LTC/SNF experience preferred. Medication administration experience. PCC experience preferred. Strong communication and patient-care skills. Available for 40 hours/week , primarily 2nd/3rd shifts. Willing to work 8- and 16-hour shifts.
