HOME HEALTH LVN - PRN
Location: Kingwood, Conroe, Spring, Humble, New Caney, Tomball, Magnolia, Montgomery

The Licensed Vocational Nurse (LVN) plays a critical role in delivering high-quality patient care within various healthcare settings across the United States. This position involves providing direct nursing care under the supervision of registered nurses and physicians, ensuring patients' comfort, safety, and well-being. The LVN is responsible for monitoring patient health, administering medications, and assisting with medical procedures, contributing to effective treatment plans. Through compassionate communication and meticulous documentation, the LVN supports both patients and healthcare teams in achieving optimal health outcomes. Ultimately, this role is essential in maintaining the continuity of care and enhancing the overall patient experience within the healthcare system.

Minimum Qualifications:

• Valid and current Licensed Vocational Nurse (LVN) license in the state of employment within the United States.
• Completion of an accredited vocational nursing program.
• Basic Life Support (BLS) certification.
• Strong understanding of nursing principles, patient care techniques, and medical terminology.
• Ability to pass background checks and meet health screening requirements.

Preferred Qualifications:

• Experience working in a clinical or hospital setting.
• Additional certifications such as IV therapy or wound care.
• Familiarity with electronic health record (EHR) systems.
• Strong interpersonal and communication skills.
• Bilingual abilities to support diverse patient populations.

Responsibilities:

• Provide basic nursing care to patients, including monitoring vital signs, administering prescribed medications, and assisting with daily living activities.
• Collaborate with registered nurses and physicians to implement and evaluate patient care plans.
• Document patient information accurately and timely in medical records to ensure continuity of care.
• Observe and report changes in patients’ conditions to the healthcare team promptly.
• Assist with medical procedures and prepare patients for examinations or treatments.
• Educate patients and their families on health maintenance, disease prevention, and post-treatment care.
• Maintain a clean and safe environment by adhering to infection control protocols and safety standards.

Skills:

The LVN utilizes clinical skills daily to assess patient conditions, administer medications safely, and assist with treatments, ensuring adherence to healthcare protocols. Effective communication skills are essential for educating patients and collaborating with multidisciplinary teams to coordinate care. Attention to detail is critical when documenting patient information and monitoring for changes in health status to prevent complications.
