How you'll make an impact in this role

• Guide patients seamlessly through their healthcare journey, acting as a dedicated navigator and advocate across the entire continuum of care to improve the patient experience.
• Bridge the gap between patients, medical staff, and community organizations, ensuring clear communication, aligned treatment goals, and a unified approach to patient care.
• Coordinate patient treatment plans and educational initiatives, championing continuous quality improvement to ensure families are informed and empowered.
• Secure essential referrals and follow-up resources for uninsured or underinsured patients, removing barriers to care and promoting health equity.
• Track patient outcomes and compile care data, transforming insights into strategic recommendations that optimize internal processes and elevate clinical delivery.

What minimum requirements you'll need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider credentialed from the American Heart Association (AHA) preferred.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of
  hire.

What additional preferences we're seeking

• Minimum of 3 years of RN experience, outpatient clinic and education experience preferred.
