Job Requirements / Qualifications

• Active Physical Therapist (PT) license in New Jersey, or eligibility to obtain NJ licensure
• Minimum 1 year of experience as a Physical Therapist; experience in assisted living, long-term care, or geriatric rehab preferred
• Strong clinical skills in mobility training, balance, strength training, and fall prevention
• Excellent communication and interpersonal skills for resident, family, and interdisciplinary team collaboration
• Current CPR certification (BLS preferred)
• Ability to document accurately in electronic medical records and complete timely assessments and discharge plans

Responsibilities

• Evaluate residents and develop individualized treatment plans to improve mobility, strength, function, and independence
• Provide evidence-based physical therapy interventions in an assisted living setting, including therapeutic exercise, gait training, and transfer training
• Collaborate with nursing, occupational therapy, speech therapy, and other care team members to coordinate care and optimize outcomes
• Document assessments, progress notes, and discharge recommendations accurately and on schedule in the EMR
• Educate residents, families, and staff on therapy goals, safe mobility techniques, and fall prevention strategies
• Participate in care conferences and contribute to interdisciplinary planning for resident-centered rehabilitation
