Therapist Physical Therapist

Job Summary:

The Physical Therapist delivers comprehensive evaluation and rehabilitation services to home health patients, ensuring personalized treatment plans that promote recovery and functional independence. This role involves supervising care delivery, collaborating with multidisciplinary teams, and educating patients and families within the home environment.

Responsibilities:

• 
  Conduct thorough assessments and develop individualized care plans tailored to patients' needs in the home setting.
  
  
• 
  Provide direct physical therapy interventions according to physician orders and established protocols.
  
  
• 
  Apply evidence-based practice guidelines, Medicare regulations, and Conditions of Participation during evaluations and treatments.
  
  
• 
  Coordinate with healthcare team members and participate in multidisciplinary conferences to optimize care strategies.
  
  
• 
  Ensure accurate documentation of all services, including treatments, adverse events, and communication in the medical record.
  
  
• 
  Supervise the activities of Physical Therapist Assistants and aides, performing supervisory visits within required timeframes.
  
  

Qualifications:

• 
  Hold a Bachelors degree or higher from an accredited Physical Therapy program and possess a current Montana PT license.
  
  
• 
  Maintain a valid drivers license, reliable transportation, and appropriate auto insurance.
  
  
• 
  Demonstrate at least two years of relevant physical therapy experience preferred.
  
  
• 
  Exhibit strong knowledge of physical therapy practices, Medicare regulations, and home health care standards.
  
  
• 
  Commit to ongoing professional development through continuing education requirements.
