Growing together.Our Hospice Chaplains provide hospice patients and families with a spiritual assessment to address spiritual, pastoral, bereavement, and additional individual counseling needs. As a Hospice Chaplain, you will also be responsible for the coordination and provision of spiritual support, as indicated in each patient's Plan of Care, on an as-needed basis.Primary Responsibilities:Work with staff, clergy, and community groups to enhance their sensitivity to the spiritual concerns of patients and families experiencing terminal illness and lossConduct initial spiritual assessments, as well as follow-up visits and contacts with hospice patients and their familiesRemain non-denominational while on dutyParticipate in patient conferences by exploring and assessing the potential spiritual needs of patients and families experiencing terminal illness and lossProvide bereavement follow-up services as defined by Southwest Medical Hospice/ Optum procedures and CHAP standardsMaintain proper records of visits with patients and familiesMake contact with clergy or appropriate representatives of patients and families, as indicatedPerform occasional liturgical assignments, such as the annual Service of Remembrance and monthly memorial services with staffDevelop and maintain a resource group of clergy to whom specific aspects of spiritual care may be delegatedProvide educational programs for community clergy, religious representatives, and lay representatives, as resources allowParticipate in a rotating on-call schedule that includes nights, weekends, and holidays (as needed/requested)You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Education, training, experience, or professional credentials appropriate to the practice of spiritual or pastoral careDemonstrated understanding of hospice philosophy, end-of-life care, grief, loss, and family dynamicsAbility to provide spiritual care that is patient-centered and respectful of diverse religious, spiritual, cultural, and non-religious beliefsSolid listening, communication, interpersonal, and documentation skillsAbility to work independently while functioning effectively as part of an interdisciplinary teamAbility to maintain professional boundaries and confidentialityAbility to meet applicable federal, state, and local requirements for the roleSuccessful completion of required background screening and hospice onboarding requirementsValid driver's license, reliable transportation, and ability to travel throughout the assigned service areaPreferred Qualifications:Degree from an accredited seminary or school of theologyCertification in hospital and/or pastoral ministryHospice experiencePastoral and/or chaplaincy experience in a healthcare settingBereavement support or counseling experienceExperience working with Electronic Medical Records (EMR) softwareComputer skills, including the ability to type and navigate a Windows environment, including Microsoft Word and OutlookPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $49,700 - $88,800 annually based on full-time employment.
