Hospice Admissions Nurse Manhattan -Bronx - Westchester

SUMMARY:  Reporting to the Patient Access Manager, and Admissions Nurse does 2-3 Admissions per day, in their territory, they are provided referrals, and it can be facility based from a hospital or nursing home, or a patients home, they confirm the patient qualifies for services, reviews medical records, and insurance, and provides patient and family education.

The Admissions Nurse is responsible for responding to and visiting patients who have been referred for admission.

The Admissions Nurse informs the patients, caregivers and family members of the hospice services available with the organization and assesses the patient for admission to services.  In addition, the Admissions Nurse is responsible for visiting patients who have been admitted to the hospital and communicates the patient status with the respective interdisciplinary team.

Is responsible for generating and responding to referrals from hospitals and other community partners. 

The Admissions Nurse will possess or demonstrate the ability to develop strong relationships with hospitals, physicians, nursing facilities and other referral sources through outreach, education and networking. 

Working with the physician and interdisciplinary team, the Admissions Nurse provides education to patients and their caregivers about hospice, evaluates for eligibility and works with the interdisciplinary team to develop an initial plan of care utilizing principles of hospital and palliative care.

DUTIES:
Appropriately evaluates hospice eligible patients in a timely manner.  Obtains approval 

from Hospice physician and the patient’s chosen attending physician to admit client to the program.  Communicates with the patient’s attending physician with notification regarding admission of the patient and care plan with review.

Promptly updates referral office/sources on the status of admissions. 
Documents information received from pre-admission visit to gather basic patient information 

and enters into the electronic medical record in a timely manner.

Notified patient care team of admissions by electronic mail to enable care to be continued by others.

As needed, arranges for transportation of patient as indicated by circumstances.
Communicates effectively and timely with the Home Medical Equipment (HME) department 

and the Pharmacist regarding patient-related equipment and medications, as needed.

Completion of initial assessment is required.
Assesses patient/family for physical, psychological, social and spiritual needs and 

collaborates with patient/family, physician, IDG to establish the plan of care, implement interventions and/or evaluate care.

Initiates and maintains communication with attending physician, Medical Director/Hospice Physician, IDG, Hospice Manager and/or Supervisor, and or other agencies as needed to coordinate optimal care and use of resources for the patient/family.

Documents according to protocol and maintains up-to-date patient records so that the care plan’s problems, interventions, actions and goals reflect baseline, current and ongoing status and changes as they occur.  Documents in home to assure timely submission of patient data to the office for inclusion in the patient record.  Syncs after every visit.

Participates in on-call or admissions assignment/rotation as directed.  

Provides education to patient and caregiver(s) as needed based on knowledge needs and learning assessment and patient/family condition.

Coordinates care with IDG and informs the Hospice Manager and IDG or unusual or potentially problematic patient/family issues.  

Works within legal, ethical practice standards to his/her position 

Code of Conduct.  Complies with appropriate policies and procedures and bring potential compliance matters to supervisor or compliance officer.

QUALIFICATION: 
Graduate from an accredited school of nursing.  RN, BSN preferred.

Current nursing license to practice profession in the state where the employee is  providing service.   

Two (2) years of experience working an acute care setting; one (1) year of hospice experience preferred.  

Working knowledge of community services.

Ability to meet, collaborate and work efficiently with others in the community.  Knowledge of entitlement programs.

Exhibits an ability and attitude relative to the spirit of caring and tradition at Hospital as required by specific job duties and function.
