About the Role:
Come join the Divine Guardian LLC team as a full-time Hospice Social Worker, where you will play a vital role in providing compassionate support to patients and their families during challenging times. Responsibilities:

• Conduct comprehensive assessments to identify patient and family needs.
• Provide emotional support and counseling to patients and their families.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Facilitate communication between patients, families, and healthcare providers.
• Assist with accessing community resources and support services.
• Document patient interactions and care plans in compliance with regulations.
• Participate in team meetings and contribute to quality improvement initiatives.
• Maintain confidentiality and adhere to ethical standards in all interactions.

Requirements:

• Master's degree in social work (MSW) from an accredited program.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of 2 years of experience in hospice or palliative care settings.
• Strong interpersonal and communication skills with a compassionate demeanor.
• Ability to work collaboratively in a multidisciplinary team environment.
• Proficient in case management and crisis intervention techniques.
• Knowledge of community resources and support services for patients.
• Flexibility to manage a varied caseload and adapt to changing needs.
