Patient Care Coordinator

The Bedford-Stuyvesant Family Health Center (BSFHC) is a Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn. We are seeking a Patient Care Coordinator to perform patient engagement services. You will be the liaison between patients and front desk and Nursing teams to ensure smooth patients’ flow and great patients experience. Essential Duties & Responsibilities Greets patients and visitors, check-in patients, ensuring a prompt, positive and welcoming experience, courteous and helpful manner Interact with patients and visitors in a professional, courteous, empathetic, and supportive way at all times. Encourage positive interactions and conversations amongst patients and staff. Transport clients to destination as needed Obtain accurate and updated patient information, such as name, address, insurance information Perform insurance verification on the date of service Assist with setting up Helow app on patient’s phones, Health Insurance issues/changing PCP, using Kiosk Facilitate and resolve patient questions regarding telehealth appointment links. Schedule appointments and manage the patient appointment calendar as assigned Collect and verify patient information, including medical history and insurance details Assist in the de-escalation of behaviorally compromised patients. Escalate the matter to management if unable to de-escalate. Performs other duties as assigned by supervisor. Act as a role model for clients and peers! Minimum Qualifications High school diploma or equivalent One (1) year of related work experience Experience Previous experience in an office or medical administrative support role is required Previous experience with insurance verification and follow-up is required Familiarity with medical terminology and procedures is a plus Knowledge of medical office operations and procedures is beneficial Skills Must be flexible with scheduling Detail oriented Great interpersonal skills Strong communication skills Be kind - we’re here to help people and provide the best possible client care Be empathetic - understand that our clients are here to get better Be emotionally intelligent - understand that clients can often be emotional over seemingly insignificant issues, and it's not personal!
