Shifts: M-F 8am-5pm, with potential on-call one day a week and one weekend a month

Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high risk members
• Collaborate with primary care providers for care plan implementation
• Address and reassess cost-efficient, appropriate levels of care
• Ensure non-medical support for treatment compliance
• Coordinate care transitions as needed
• Engage specialty resources for optimal resolution
• Maintain detailed records of outcomes during management
• Identify health promotion and illness prevention opportunities
• Act as patient advocate, protecting privacy and confidentiality

Requirements of the RN - Case Management - Travel

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted CO Registered Nurse license
• Ability to analyze complex medical information and make rational decisions
• Case Management certification
